Merit badge Class Requests – Please mark attached schedule with first and second choices – Due Wed, June 17, 2009 2. Health forms – Due Wed, Jun 17, 2009 a. The Annual Health and Medical Record this replaces the Class 1, 2, and 3 forms and will become mandatory in 2010 b. Class 1 - must be current and include a copy of the Scout’s health insurance card. c. Class 2 – must be no older than 36 months as of June 28. This is good for Scouts and adults under age 40. d. Class 3 – must be completed for adults 40 and over. 3. Specific permission for Scout to participate in extracurricular activities – TBD in meeting Packing Requirements Personal Hygiene (please not fragrance – it attracts bugs) 1. Soap 2. Shampoo, comb/brush 3. deodorant 4. shaving needs, if applicable 5. towel 6. flip flops or bath shoes to be worn in shower or on boat (not in camp) Clothing: 1. Class “A” Uniform – that is shirt, neckerchief, pants/shorts 2. Class “B” t-shirt 3. Swim suit and beach towel 4. closed toed shoes and spare pair 5. raingear 6. clothes and underwear for 7 days 7. water-proof hiking shoes or boots 8. jacket 9. extra socks Sleep and Comfort 1.
